Confidentiality & Medical Records

Both your visits to Health Services and your medical records are regarded as privileged communication and are regulated by strict policies to protect confidentiality. Health records are maintained in compliance with federal and Missouri state laws. Health Services staff members are committed to upholding your right to privacy.

Information from records will not be presented to anyone without your written authorization, except as required by law. In order to protect your rights, please do not be offended if identification is requested prior to release.

Your student/client rights and responsibilities at Health Services

Effective health care includes certain rights and responsibilities and requires a special relationship between the health care provider and the client. This agreement contributes to more effective treatment and greater client satisfaction.

You have the RIGHT to:

  • be treated with dignity and respect
  • know the names and professional status of those serving you
  • privacy
  • confidentiality of information in your records
  • receive accurate and understandable information about your health
  • be seen within a reasonable amount of time, giving consideration to emergencies and critical illness
  • know the significant beneficial effects, side effects and problems of treatment
  • participate in choosing a form of treatment within the scope of services of Health Services
  • coordinated transfer of care to another health provider or facility when you have agreed to such a move and the referral site has accepted you for admission and referral
  • receive education and health counseling
  • consent to any care or treatment; or to refuse care or treatment after being informed of the medical consequences of refusal and having signed a release
  • be informed about services and any related costs
  • receive appropriate referrals to other providers and services

You also have the RESPONSIBILITY to:

  • present accurate identifying information
  • seek medical attention promptly
  • give accurate information about your medical history
  • ask about anything you do not understand
  • report any significant changes in symptoms or failure to improve
  • respect Health Services policies
  • treat all personnel and health care professionals with courtesy and respect
  • return borrowed equipment within the specified time
  • seek non-emergency care during regular hours
  • to provide useful feedback about our services and policies
  • to know the names, purposes and effects of any over-the-counter medications given to you
