The health packet must be completed in order for you to participate in clinical practice. The forms are due September 1.

Please be advised the following are mandatory for you to practice in clinical rotations:

  • Drug screen with chain of custody performed by Quest Diagnostics as per the Nurse Anesthesia program. You will need a lab request from Student Health Services.
  • Health insurance - coverage must be continuous until August of the following year.
  • Hepatitis A - 2-dose series
  • Hepatitis B - 3-dose series
  • Meningitis - Within the past three years
  • MMR - 2-dose series
  • Physical exam - Within the past 12 months
  • Polio vaccination - 3-dose series
  • Tetanus/duphtheria - Administered after Aug. 1, 1998
  • Tuberculosis test (2-step method) - 2-dose series; or documented history of the disease
  • Varicella (Chicken Pox) - 2-dose series; or documented history of the disease
