All new residential students are required to submit the following documents PRIOR to your move in date:

• Webster University Tuberculosis Screening Form
• Immunization Record
• United Healthcare Student Resources Student Injury and Sickness Plan Enrollment Form
• Copy of both sides of your health insurance card (domestic residential students only)

Failure to return these documents and forms to the Student Health Services Department PRIOR to “move-in-date” will result in a hold to be placed on your student account, registration and you will not be able to enter campus housing.

Student Health Insurance Reminders:

• Domestic residential students must submit a copy of both sides of their health insurance card if planning to waive the Webster University Student Health Insurance Plan. 
• International Students may not waive the Webster University Student Health Insurance Plan.
• Please review the Student Health Insurance Program on the Webster University website for insurance information.
